Jurong Community Hospital (JCH) was built together with Ng Teng Fong General Hospital (NTFGH) from the ground up as one integrated healthcare hub. The twinned hospitals were designed to deliver seamless patient-centred care, providing the right care at the right place and at the right time. Siting JCH adjacent to NTFGH allows for convenient and safe transfer of patients from acute care to post-acute and continuing care facilities. The co-location of both hospitals allows for better coordinated and integrated care as patients will benefit from the co-management by doctors from NTFGH and JCH.
JCH is also the first community hospital in Singapore to feature a LIFE Hub and Mobility Park for patient education and rehabilitation. LIFE Hub aims to help seniors remain healthy and independent at home. It features a three-room HDB flat mock-up, which serves to help patients ease back into their home environment under the guidance of our therapists. It will showcase a variety of products and provide practical solutions that patients can use to help them live more safely and independently in the home setting. It will also offer an information hub and educational area for patients and their caregivers. A first in Singapore, the Mobility Park simulates a public transport setting featuring an actual MRT carriage, bus and taxi, complete with ramps and steps. It provides patients with a safe environment to re-adjust to taking public transport and improving confidence to go out on their own as they prepare to return to their daily lives.

Woodlands Health (WH) is the newest integrated acute and community hospital serving the population in the north-western part of Singapore. It will have up to 1,000 acute and community beds and almost 400 beds in the long-term care facility and offers a full suite of clinical services. As a fully integrated campus with acute and community beds sharing the same building and facilities, patients can seamlessly transit to the different care settings for better patient experience and clinical care.
WH has been designed to improve overall operational efficiency and clinical effectiveness. This starts from the design of the emergency department that enables effective and efficient patient triaging based on their acuities to the direct connection to radiology and cardiology services to the operating theatres and directly to the intensive care units. To optimise rehabilitation outcomes, facilities have also been designed and nursing teams in the wards have also been empowered and trained to enable patients to continue to under rehabilitation in the wards between rehabilitation sessions. Woodlands Health is also the first public healthcare hospital to be fully integrated with National Parks, with the largest therapeutic garden designed to support patient’s rehabilitation and recovery journey.
WH has also designed novel and integrated care models to support patient care. Some examples include the Advanced Practice Musculo-skeletal service in the Emergency Department, Integrated Allied Health services where the same Allied Health professional follows the patients along their journey across different care settings and trans-disciplinary Allied Health services such as Aural Rehabilitation services.

Khoo Teck Puat Hospital (KTPH) is committed to deliver person-centred care that fosters greater dignity and compassion, with a particular emphasis on empowering vulnerable and dependent individuals. In recent years, KTPH has introduced several notable initiatives, including Humanitude care, Garden by the Bed, and Eldercare Companions.
Humanitude is a relationship-centred and compassionate care approach. Humanitude has been evidenced to reduce agitation and resistance to care in people with dementia and improve their acceptance of care, participation, mobility and well-being. Furthermore, Humanitude can improve care communication, empathy, job satisfaction and reducing burnout among healthcare professionals.
Garden by the Bed is an initiative by a team of occupational therapists to bring horticultural therapy to patients’ bedsides. This project involves collaboration with dedicated volunteers who help in providing and maintaining the plants in the wards. The program has demonstrated effectiveness in enhancing the mood, engagement, quality of life, mobility, and overall hospital experience of older patients.
Eldercare Companions program aims to address the socio-emotional needs of patients with dementia, delirium, and depression in the hospital. KTPH has recruited dedicated individuals from the general public, many of whom are mature in age, possess strong interpersonal skills, and can connect well with patients. They devote their time and attention to enhancing the emotional well-being of the patients.

Centre for Innovation in Healthcare
The NUHS Centre for Innovation in Healthcare (CIH) serves as a comprehensive hub dedicated to the clinical adoption of Practice Changing Innovations (PCI) for the health system of the future. By leveraging the three key assets of NUHS—patients, healthcare professionals, and clinical data—CIH supports innovators in demonstrating the clinical value of their breakthroughs, with the overarching goal of transforming care delivery and improving population health. While primarily focused on NUHS assets, CIH acknowledges the importance of partnering and collaborating with stakeholders across the healthtech ecosystem to effectively serve as a one-stop hub.
The PCI programme is a flagship initiative of NUHS. Through this CIH-supported programme, rigorously selected home-grown innovations with the potential to significantly impact clinical practice are rapidly deployed within NUHS, with plans for eventual scaling both nationally and globally. Additionally, CIH offers clinical services to selected innovators outside the PCI programme, facilitating not only clinical testing within NUHS hospitals and polyclinics but also the deployment of these innovations.
CIH has established a dynamic co-innovation community, enabling both local and international innovators to expedite their journey towards clinical adoption. The centre also contributes to talent development efforts, fostering a culture where “Everyone is an Innovator.” By mentoring students from various faculties and institutions, as well as aspiring clinician innovators, CIH provides a supportive environment for the development and advancement of new ideas and inventions.
Through its multifaceted approach, CIH actively advances high-potential PCIs, enabling NUHS to consistently deliver the highest quality care to patients and fulfilling its mission of improving health and care for Singapore and the region.

Singapore National Eye Centre (SNEC)’s site visit will showcase various allied health professions specialising in the field of ophthalmology, at unique sites such as the eye bank, optometry and orthoptic departments. We will provide an overview of our diverse services at SNEC, with a focus on our roles in visual rehabilitation. A fun, hands-on session teaching transferable vision screening skills, such as how to perform standardised vision and visual field screening tests, will be organised to help all allied health professionals better understand and manage the visually impaired. This transdisciplinary sharing will be helpful for anyone keen to learn about how to identify and guide visually impaired patients during their daily course of work, enhancing patient outcomes and for appropriate, timely referrals to be made when necessary.
